Gather Your Documents
Having the right paperwork ready before you start your application is the secret to getting verified quickly.
1. Proof of Identity
You must prove that you are who you say you are. The name on your ID must match your application exactly.
Accepted Items (Choose One):
- Valid Driver's License (U.S.)
- State-Issued ID Card
- U.S. Passport or Passport Card
- Tribal Identification Card
- Permanent Resident Card (Green Card)
- Government Assistance Program Card (if it has your photo)
⚠️ Vital Tip: No Blurry Photos
When you take a picture of your ID to upload, make sure there is no glare from the flash. The text must be clearly readable, and all four corners of the card should be visible.
2. Proof of Eligibility
You need to prove EITHER that you are in a program OR that your income is low enough.
Option A: Program Letter
If you qualify through SNAP, Medicaid, or SSI, you need an Award Letter or benefit statement.
- Must be issued within the last 12 months.
- Must clearly show your name.
- Must show the name of the program (e.g., "Medicaid" or "SNAP").
- Must have the effective date or expiration date.
Option B: Income Proof
If qualifying by income, you need documents showing your annual household earnings.
- Prior year's State or Federal Tax Return (Front page).
- Three consecutive months of Pay Stubs.
- Social Security Statement of Benefits.
- Veterans Administration Statement of Benefits.
3. Proof of Address
(Only needed if your ID has a different address)
If your Driver's License has your old address, you need a secondary document to prove where you currently live.
- Current Utility Bill (Gas, Water, Electric)
- Current Mortgage or Lease Statement
- Most recent W-2 or Tax Return
